The back cover of the Participant Education Booklet includes a pocket into which Enrollment Forms can be placed. PlanToACT can print all Enrollment Forms needed such as Investment Election Forms, Summary Plan Descriptions, Beneficiary Designations, etc. PlanToACT also can furnish all prospectuses on a CD along with the proper disclosures to satisfy electronic delivery per Department of Labor regulations.
